Please list any allergies, health concerns, or other important information we should know about your child. Please make sure your child's teachers and the school Director understand the severity of any allergies.
Symptoms displayed when having an allergic reaction:

*Summer Camps are both non-refundable and non-transferable.

*No refunds will be made for sickness or missed days, and we will not be able to make up missed days in another week.

*Please verify when your child starts school before signing up for the last 2 weeks of school. Some schools begin August 1. Registrations are non-refundable.

*All children enrolled at The Spanish Academy are required to have health insurance.

*Candid pictures and video of children at The Spanish Academy may be used for advertising purposes unless the administration has been notified otherwise.

*In order to mitigate risk and to help avoid school closures, I agree to be honest and transparent about any illnesses in my family.

*Should my child suffer an injury or illness while in the care of The Spanish Academy, and the school is unable to contact me (us) immediately, it shall be authorized to secure medical attention and care for my child as may be necessary. I (we) assume responsibility for payment of services. I (we) agree to keep the school informed of changes in telephone numbers where I (we) can be reached.
